Bridges to Transition

Bridges to Transition is designed to provide support, training, and resources needed for the successful transition of individuals with autism who are leaving their educational entitlements (on or before age 21).

Bridges to Transition will “bridge” the gap in existing services and help individuals and families envision what vocational, employment and/or day supports will be needed after age 21.

A unique feature of “Bridges to Transition” is personalized planning with individuals and families, as well as education about adult service delivery systems, and available resources, to help make informed decisions that best meet individuals’ needs.


How do I know this program is right for us?

  • Does your child have a solid transition program in place for when he/she ages out of their educational program?

  • Are you familiar with NJ state funding resources and processes to obtain funding?

  • Is your young adult seeking a day program or self-funded program?

  • Will your young adult need housing?

  • Is your young adult seeking job placement?

  • Have you secured necessary and available funding to meet your young adult’s needs?
